All-on-X

All-on-X describes a full arch prosthesis supported by a variable number of dental implants.

The X stands for the implant count, which varies with the patient's anatomy, bone volume, and the clinician's plan. Written out, the family of terms includes All-on-4, All-on-5 and All-on-6, and All-on-X is the general form used when the count is not being specified.

The number is a clinical decision, not a marketing tier. A higher implant count is not automatically a better outcome, and the appropriate count for a given patient belongs to the treating clinician and the imaging in front of them.

We use the term in marketing because it is what patients search for. We do not use it to imply a specific product, a proprietary system, or an outcome.

What is included and what is excluded

Counts

  • Full arch prostheses supported by a stated or unstated implant count
  • The specific variants commonly written as All-on-4, All-on-5 and All-on-6
  • Both upper and lower arch treatment

Does not count

  • Single tooth and partial implant restorations
  • Conventional dentures with no implant support
  • Any claim about which implant count suits a given patient
  • Any claim of a specific clinical outcome or longevity

A worked example

Three patients are planned for full arch treatment in the same week.

  1. 01Patient one, upper jaw, 4 implants: All-on-4
  2. 02Patient two, lower jaw, 6 implants: All-on-6
  3. 03Patient three, both jaws, 5 implants each: All-on-5, two arches
  4. 04Arches treated: 1 + 1 + 2 = 4

Three patients, four arches, and three different implant counts, all All-on-X.

How this differs from

  • Full arch

Full arch is the scope restored. All-on-X is the implant supported approach.

  • Lead

All-on-X is usually the search term. A lead is what happens when somebody searching it makes contact.

  • Presented

The implant count is decided clinically before a plan and fee are presented.

These definitions come from our published measurement standard. Read The Arch Standard for the reporting rules they sit inside, or see every term.
